Apple Watch has reported a single heart rate variability number since 2017. As of the Series 12 and Ultra 4, it reports two, and the new one is not the metric the watch has been using for the last nine years.
Apple calls them Recovery HRV and overall HRV. Recovery HRV is described as best for identifying daily signals of stress and recovery. Overall HRV covers broader health, including cardiovascular health. Both come from the new Health Sensing System, which measures HRV as often as every five minutes, or up to 24 times more frequently than the Series 11 and Ultra 3 managed.
The detail Apple did not put in the keynote is more interesting than the division itself.
HealthKit Quietly Gained a Second HRV Type
Alongside the launch, iOS 27 and watchOS 27 added a new HealthKit quantity type called heartRateVariabilityRMSSD. It is the only new quantity type among the 122 in this release, and it matters because of what came before it.
HealthKit has carried exactly one HRV type since iOS 11 shipped in 2017: SDNN, the standard deviation of NN intervals. Every Apple Watch HRV reading you have ever seen, in Apple's own apps and in every third-party app that reads Apple Health, has been SDNN.
RMSSD is the root mean square of successive differences. It is the metric most of the rest of the wearable industry standardized on years ago, and HealthKit had no way to represent it until now. Apps that computed RMSSD from another device had nowhere to write it.
The Two Metrics Are Not Interchangeable
This is the part worth understanding, because it explains why Apple is presenting two numbers instead of one.
SDNN measures the spread of all your normal heartbeat intervals across the recording window. Because it captures total variability from every source, it includes slow-moving influences like circadian rhythm, body temperature, and how active you have been. Its interpretation depends heavily on how long you measure. SDNN from a 24-hour recording and SDNN from a five-minute reading are not comparable numbers, even though both are called SDNN.
RMSSD measures the change between each consecutive pair of beats, squares those differences, averages them, and takes the square root. Working on beat-to-beat change rather than the spread of a whole recording, it mostly reflects parasympathetic activity, the branch of your nervous system associated with rest and recovery. It is the more reliable metric over short recordings, which is exactly what a watch takes.
The practical consequence is blunt: the two values are not interchangeable, and a single night can move them in opposite directions. If you have been comparing your Apple Watch HRV to a Garmin or Whoop number, you have most likely been putting SDNN next to RMSSD and wondering why they disagree.
What Apple Has Not Said
Here is where the story stops being clean.
Apple has not confirmed that Recovery HRV is RMSSD and that overall HRV is SDNN. The mapping lines up with how those two metrics are conventionally used, and adding RMSSD to HealthKit in the same release that rebuilds HRV measurement points strongly in that direction, but Apple has published no mapping. Until either the documentation or the data on device settles it, treat the correspondence as an inference.
Apple has also not published the algorithm behind Recovery HRV, the window it is calculated over, the units or ranges it reports, how many nights build the personal baseline, or how far a reading must sit from that baseline before anything gets flagged. Overnight Vitals compares Recovery HRV against your personal baseline, so the number is judged against your own history rather than a published range. That is the right way to do it, and it still leaves the mechanics opaque.
Who Actually Gets It
This is hardware, not software, and that distinction is doing a lot of work.
The five-second heart rate sampling, the five-minute HRV cadence, the two HRV variants, and the Readiness score are all tied to the new optical and electrical heart sensors and the more powerful LEDs in the Health Sensing System. They apply to the Series 12 and the Ultra 4 only.
watchOS 27 does run on the Series 9 and later, the SE 3, and the Ultra 2 and later, and it arrives free on September 14. What those watches get are the software features: Siri AI with personal context, perimenopause and menopause support in Cycle Tracking, Workout Buddy without a nearby iPhone, and a single-handed tap gesture for the Smart Stack. None of the sensor-dependent health features are on that list, and Apple's compatibility footnotes do not mention them for older models.
So an Ultra 3 owner running watchOS 27 gets a better watch in several ways and does not get Recovery HRV, overall HRV, or Readiness. That is a real upgrade gap for a watch that was the flagship a year ago.
What to Watch Next
Two things will settle the open questions, and neither is in Apple's control.
The first is developer documentation catching up. If Apple documents what each HRV type contains and which one each app view uses, the mapping question closes.
The second is measured behavior. Existing apps need a new authorization request to read RMSSD, since permission flows do not include the new type by default. There is also no historical backfill, so an RMSSD series starts from the moment the watch begins writing it. That means the practical way to find out what Apple Watch actually writes is to wear a Series 12 for a full day, then compare the RMSSD and SDNN streams against what the Heart Rate app shows.
Bottom Line
Apple Watch went from one HRV number to two, and the new one is a different metric with different behavior. If you use HRV as a recovery signal, the important takeaway is that your history is SDNN while the new Recovery HRV reading is almost certainly RMSSD, and those two things do not track each other night to night.
That matters most if you own a Series 12 or Ultra 4, because you now have two numbers to interpret instead of one. It matters just as much if you own an Ultra 3 or Series 11 and were expecting the new health features to arrive with watchOS 27. They are not coming.
