Apple Watch Hypertension Alerts Reach the Philippines, but Not on the Newest Watches

Apple turned on hypertension notifications for Apple Watch in the Philippines in a newsroom release dated October 14, and the most interesting detail in the announcement is a footnote.

The feature works on five Apple Watch models there. It does not work on the two newest ones.

Apple said so directly in its own Philippine newsroom post: hypertension notifications are available today on the Series 9, Series 10, Series 11, Ultra 2 and Ultra 3, and while the newer watches get everything else Apple shipped this year, this one health feature is still stuck in a regulatory queue.

What Apple actually announced

The Philippine release follows the same script Apple has used in every market where the feature has landed. It describes an alert that watches for signs of chronic high blood pressure using the optical heart sensor, running passively in the background over 30-day windows, and notifies you when a pattern shows up.

Apple also repeats the framing that matters most here: this is not a blood pressure measurement. If you get a notification, Apple recommends logging your blood pressure for seven days with a third-party cuff and taking the results to a clinician, which matches current American Heart Association guidance for diagnosing hypertension.

The company adds the numbers it usually includes for this feature. The algorithm was trained on data from studies totaling more than 100,000 participants and then validated in a clinical study of over 2,000 people. Apple is upfront that it will not catch every case.

Then comes the part worth reading twice:

Hypertension notifications will not be initially available in the Philippines on Apple Watch Series 12 or Apple Watch Ultra 4, as additional regulatory clearances for the feature on these models are in process; availability for these models is expected next year.

Who is left out, and why

Here is the split, straight from Apple's release and its support documentation:

  • Series 9, Series 10, Series 11, Ultra 2, Ultra 3 get hypertension notifications now
  • Series 12 and Ultra 4 do not, and Apple says availability is expected next year
  • The same warning appears on Apple's support page for the feature, so it is not a Philippines-only quirk

The reason is hardware, not strategy. The Series 12 and Ultra 4 introduced a rebuilt Health Sensing System, with larger ECG electrode coverage, redesigned photodiodes in a ring layout and larger green LEDs that let the watch sample heart rate every five seconds. New sensors mean the algorithm runs on different hardware, and the clearances that covered the previous generation do not automatically carry over. We walked through the sensor changes in our Series 12 and Ultra 4 teardown coverage.

So the newest Apple Watch is the one that cannot run the feature in the Philippines today, while watches going back to 2023 can.

This is a rollout, not a single launch

The Philippine release is one step in a march Apple has been making market by market. Apple has published near-identical "available today" posts for Canada in October 2025, India in December 2025, Australia in January 2026 and Israel in May 2026.

That pattern is the reason we flagged the exclusion problem when the Series 12 shipped. Apple's watchOS feature availability notes carry a footnote for 33 countries and regions warning that the feature may not work on the Series 12 or Ultra 4 until clearances land, and the United States, the UK and EU markets are not on that list.

Countries have also been cleared for the feature itself at different speeds. Sleep apnea detection followed a similar path, including a China-only clearance that took a separate regulatory route, which we covered in our sleep apnea rollout piece.

What this means if you are buying in the Philippines

If you already own a Series 9 through Series 11, or an Ultra 2 or Ultra 3, the feature is available now and you can set it up from the Health app under Health Checklist. You need Wrist Detection on, an iPhone 11 or later, to be 22 or older, not pregnant and not previously diagnosed with hypertension, because the feature is designed to catch the undiagnosed.

This is the part where the old watch is genuinely better than the new one. Trading up to a Series 12 or Ultra 4 in the Philippines currently means giving up a health feature you already have, until Apple's clearance for the new sensors comes through.

If you are shopping for a supported model, the Apple Watch Series 11 on Amazon is the newest watch that runs hypertension notifications today, and a basic upper-arm blood pressure monitor is what Apple's own guidance tells you to use if an alert does arrive.

The rest of the feature is unchanged wherever you live. It is a screening prompt, not a diagnosis, and it is built for people who do not know they have a problem.

Bottom Line

Hypertension notifications went live on Apple Watch in the Philippines on October 14, on the Series 9, Series 10, Series 11, Ultra 2 and Ultra 3. The Series 12 and Ultra 4 are excluded because the rebuilt sensor package needs its own regulatory clearance, and Apple says that is expected next year. For buyers there, the short version is simple: the newest watch is the one that cannot do this yet, and upgrading from a supported model takes the feature away until the paperwork catches up.
